Year 9: Missed Td/IPV & MenACWY Vaccinations

If your child has not yet received the above vaccines, the School Aged Immunisation Team is offering another opportunity for Year 8 / Year 9 children to receive the relevant vaccines in school on the above date.

If you would like your child to receive the vaccine, please click the Year 8 or Year 9 blue button below to complete the NHS online consent form. If you would like to decline the vaccine for your child, please complete the online form to provide ‘NO TO CONSENT’.

The online forms will close on 16 July 2025. Paper consent forms are also available from school.

The Secondary School Immunisation & Vaccination Team comes into the Academy throughout the year to provide vaccinations to our students.
